If you would like to appeal a library fine, please complete the form below, and then submit it within 60 days of the charge notification from USEK Library. The appeal decision will be made within ten (10) working days of filing the form, and you will be notified by e-mail.
All decisions are final.
This is only an appeal and not a waiver of payment.
To pay your fine, you can stop in at the Library Circulation Desk during opening hours.
This appeal is for current fines only. Charges on lost and/or damaged items may not be appealed.
